HOSPICE CARE: WHAT TO EXPECT
- After the required documents are signed and admissions orders are received, the patient is officially admitted to hospice care.
- Staff visits are scheduled accordingly.
- All required medical equipment and medication for symptoms will be delivered. The hospice team communicates with the patient’s physician and the hospice physician to discuss medical history, current physical symptoms, and life expectancy.
- On staff, social worker, and a chaplain site visit to augment holistic and psychosocial exams to the patient’s plan of care.
- Provide information to family to connect with the Lion Heart staff regarding questions scheduling a visit.
ELIGIBILITY / QUALIFICATIONS
Must be prescribed by a physician to obtain services Medicare/Medicaid or other private/commercial medical insurance reimbursements.
